Roles & Responsibilities

• Patient Assessment

  Conduct comprehensive assessments of patients' health status, including physical, emotional, and social aspects, to determine appropriate care plans and interventions tailored to individual needs.

• Care Plan Development

  Collaborate with patients, families, and multidisciplinary teams to develop individualized care plans, setting measurable goals and updating as needed to ensure optimal patient outcomes.

• Medication Management

  Administer prescribed medications, educate patients on proper usage and potential side effects, and monitor adherence and response to therapy to ensure effective treatment.

• Wound Care Management

  Provide specialized care for patients with wounds, ensuring proper cleaning, dressing, and monitoring of the healing process, while educating patients and caregivers on best practices.

• Health Education

  Educate patients and their families on self-care techniques, disease management, and preventive measures to enhance wellness and reduce hospital readmissions.

• Documentation and Reporting

  Maintain accurate, timely documentation of patient care activities, observations, and responses, complying with healthcare regulations and contributing to quality improvement efforts.

• Patient Advocacy

  Act as a liaison and advocate for patients' health needs and preferences during interactions with healthcare providers and family members, ensuring respectful and holistic care delivery.

Typical Required Skills and Qualifications

• Registered Nurse (RN) license with a minimum of 2 years in a clinical setting

• Experience in home health care or community health is highly preferred

• Strong assessment and critical thinking skills

• Excellent communication and interpersonal abilities
